What is the Acknowledgment of Risk Form?
In addition to the data sheet and release forms that are a part of the StoryCorps signature recording process, there is now an Acknowledgment of Risk form and a Health Assessment form. These forms will need to be completed by the time your interview begins.

If there are one or more participants who prefer not to complete these forms, you should ask to change your in-person appointment to a virtual appointment. You can do so by calling 1-800-850-4406.
How can I fill out the form?
StoryCorps can provide you with a tablet to fill out the form, a link to the form via email, or a link to the form generated by a QR code. You can also receive a copy of the form in hard copy. Please let us know if you would like to have the form translated into a language other than Spanish.
Why do I have to complete this form?
COVID-19 is a current health crisis, and this form highlights what you can expect from us and what we expect from you in order to make your interview experience as safe as possible. Despite taking safety precautions, StoryCorps cannot eliminate 100% of the risk of exposure to COVID-19.This form ensures that you are aware of the exposure risks, including travel to and from the interview location. By signing this form, you agree to not hold StoryCorps responsible for any exposure to COVID-19 that may occur during your appointment.
Other Important Things to Know:
StoryCorps has the right to cancel or reschedule a StoryCorps interview for any reason and at any time, including non-cooperation with StoryCorps COVID-19 protocol.
For minors (anyone under 18) participating in a StoryCorps in-person interview, a parent or guardian must sign the Acknowledgement of Risk and Health Assessment Forms on their behalf. If a parent or guardian will not be the adult accompanying the minor to the interview, please contact us to arrange to sign the forms in advance of your appointment at email@example.com.
